Hospital Outreach Team

Who are we?

We are a multi-professional team working in partnership with your GP, District Nurse and other Health Care Professionals.We work for the Greenwich & Bexley Cottage Hospice and provide a Specialist Palliative Care Service within your own home:

Specialist advice, information and support to you and your family.
Help with any symptoms such as pain, nausea etc. by advising you and your primary care team.
Help for you and your family in adjusting to your diagnosis and the changes you may experience in the course of your illness.
Education and support to your primary care team.
Helping you to make decisions around your care.

The aim of our service is to work with you and the professionals involved in your care to help with complex pain and other symptoms as well as give psychological, social and spiritual support. Our goal is to achieve the best quality of life for you and your family.

The Team

The team consists of Clinical Nurse Specialists, A Specialist Psychosocial Worker, Palliative Care Doctors, Counsellors and Secretarial support.
