Support Services - Community Liaison

Many potential patients and carers are unsure what the Hospice offers and whether it is appropriate for them.

To enable the community to have access to all aspects of the Hospice service we have a designated specialist nurse who occupies the post of Community Liaison Nurse, who will:

  •  Visit patients who have recently been referred to the Hospice for day care or as In-   patients
  •  Through this visit she will be able to discuss with the patient and their carers what is offered at the Hospice
  •  Make an assessment and advise on how best we can help the patient and carer.
  •  Refer to other services provided – such as counselling, the Lymphoedema Service and Breathlessness Clinic.
  •  Speed up admission if appropriate.
  • Understand the family background in preparation of discharge.
