Clinical Nurse Specialist - Hospice Outreach Team – Community

Salary: Agenda for Change Band 6/7 Depending on eperience, GBCH terms and conditions

This team provide specialist care, support and advice to patients with life limiting illness and their families as well as other health and social care professionals involved in their care.  Due to service development and staff changes we require senior nurses to join the team
