We are developing clinical services to improve the care provided to patients with life limiting illness, their families and carers in the London Boroughs of Greenwich & Bexley.  We now have the following opportunity available:

Discharge Co-ordinator - Hospice Outreach Team - Hospital

Salary: Agenda for Change Band 6 (Fixed term contract/secondment 2 years)(37.5 hours)

This team provides specialist support and advice to staff, patients and their families requiring palliative care in Queen Elizabeth Hospital, Woolwich. Due to additional investment by NHS Greenwich the following post has been developed to facilitate patients with complex needs to achieve their preferred place of care. m
