We are developing clinical services to improve the care provided to patients with life limiting illness, their families and carers in the London Boroughs of Greenwich & Bexley.  We now have the following opportunity available:

End of life Care Support Nurse - Care Homes Support Team

Salary: Agenda for Change Band 6 (Fixed term contract/secondment 2 years)(37.5 hours)

Due to additional investment by NHS Bexley, the Hospice is expanding the service we currently provide to care homes, supporting them in the implementation of the End of Life Care Strategy. We require the following person to join our Care Homes Support Team.m
